Handling Duplicate Tax Slips

Submit a request when there is a duplicate Tax Slip

First, find the client's CRM Contact Record page by searching contact name in the search bar.

From the client's contact record, start by navigating into the Servicing (Service Request) tab, which is found in the middle panel of the contact record. Select Account Administration

From there, you can select Special handling and proceed to specify the details of the request via the right sidebar pop-up. 

 

Step 1. 

  • Select Tax Slip Duplicate 

Step 2. 

  • Select Account that you received the duplicate tax slip for

 

Step 3.

You will be promoted to specify: 

  • Fiscal Year: The drop down arrow will allow you to pick the year.
  • Tax Slip: You can choose which tax slip needs the corrections based on the account type:
    • Life Income Fund (LIF): T4RIF, NR4, or Other. 
    • Cash or Registered Education Savings Plan (RESP): T5, T3, T5013, NR4, 1099-INT, 1099-DIV, 1099-B, 1042-S, or Statement of Securities Transactions
    • Tax-Free Savings Account (TFSA): T5, T3, T5013, NR4, 1099-INT, 1099-DIV, 1099-B, 1042-S, or Statement of Securities Transactions
    • Registered Retirement Savings Plan (RRSP): T4RSP, RRSP Contribution Receipt, NR4, or Other
    • Registered Retirement Income Fund (RIF): T4RIF, NR4, or Other
  • Additional Comments: If there are any comments you need to add. 
  • Attached Documentation: For this request you will need to attach a Tax Slip or Proof of Transaction. 

Step 4. 

  • Click Submit once the files are fully uploaded. You can now track your ticket.

Tax Slip Duplicates requests are generally completed in 10 business days
