LIRA/LIF With Successor Holder (Must Be Spouse)

If the surviving spouse is the successor holder of a locked-in account, you must attach these documents to your estate settlement request

Required Documents:

1: Declaration and Instructions for Settlement of Locked In Account (Form 26819-202)

  • Please fill out this form and attach it to your estate settlement request.
  • Ensure that a Commissioner of Oaths or Notary signs (bottom left) or stamps the form. 
  • A notary public is a person who witnesses oaths, signs affidavits, and certifies documents to be true copies of the original.
  • A Commissioner of Oaths has the authority to administer oaths when you sign an affidavit or statutory declaration. 
  • Documents requiring a Commissioner of Oaths and Affidavits can be commissioned in the City Clerk’s Office.

3: Portfolio Statement as of Date of Death
