How to Complete the Power of Attorney (POA) Panel
Learn how to accurately complete the Power of Attorney panel when opening a new account.
The Power of Attorney (POA) panel collects essential personal details about the account holder power of attorney, such as personal details, address, employment information, investor details. Appointment of a POA, entered at the account level if the client wishes to assign one.
If the client’s details already exist within the CRM, the information in this panel will auto-populate. You must still open the panel to review, verify accuracy, and click Save to mark it complete.
Note: You only need to fill this panel out if you require a POA to sign the application for them.
Filling Out Power of Attorney (POA) Information
Enter the client’s Power of Attorney details into the provided fields:
- Does the client wish to add a power of Attorney: In this field select yes or no.
- Select Power of Attorney: You can either select a contact already in the system or create a new contact as POA. Once selected you will need to enter the following information:
Filling Out POA Personal Information
- Title: Select the appropriate title (e.g., Mr., Mrs., Ms.).
- Gender: Select the client’s gender (Male, Female, Other).
- First Name: Enter the client's first name.
- Middle Name: Enter the middle name, if applicable; otherwise, leave blank.
- Last Name: Enter the client's last name.
- Primary Phone Number: Enter the client’s main contact number (required).
- Secondary and Other Phone Numbers: Enter additional contact numbers if applicable (optional).
- Date of Birth: Enter in YYYY-MM-DD format exactly as shown on the client’s ID
- Marital Status: Choose the client's current marital status from the dropdown.
(Selecting "Married" will activate additional required fields in the Spouse panel.) - Relationship with Account Holder: Indicate the relationship that the POA has with the account holder.
Filling Out POA Address Information
- Civic Number: Type in the civic number.
- Street Name: Type in the street name
- Street Type (if applicable): Type in the street type. (E.g., Dr, Avenue, Way).
- Unit Type (if applicable): Type in the unit type. (E.g., Apt, Unit).
- Unit Number (if applicable): Type in the unit number.
- Street Direction (if applicable): Type in the street direction. (E.g., North, SW,).
- Country: Type in the country. (E.g., Canada, United States).
- City: Type in the city. (E.g., Toronto, Oakville)
- Province: Type in the province. (E.g., Ontario, Alberta.) If you are in the U.S then type in the state. (E.g., New York, Ohio)
- Postal Code: Type in the postal code. (E.g. for postal code X#X#X#) (E.g., for zip code ##### or #######)
Filling Out POA Employment Information
- Employment Status: Enter the employment status. (E.g., self-employed). If retired is selected then an additional field of former employer and former occupation / Position will be required.
- Employer Name: Enter the name of the client's current employer.
- Employer's Address: Enter the address of the employer.
- Nature of Business: Enter the nature of the business. (E.g., Landscaping)
- Occupation / Position: Detail the clients's current occupation or role. (E.g., Owner)
- Years Until Retirement: Enter the number of years until the client plans on retiring.
Filling Out POA Investor Details
- Is the client considered a reporting insider within the meaning of the regulation?: Yes or No

Does the client, as an individual or as part of a group, hold a controlling interest in such a company (more than 20% of the outstanding voting securities of an issuer)?: Yes or No

Is the individual with authority or any of their family members with whom they reside, an employee, officer or director of a securities dealer?: Yes or No

Is the client, a member of their family, or a close associate of theirs a Foreign Politically Exposed Person (PEP)?: Yes or No

Is the client, a member of their family, or a close associate of theirs a Domestic Politically Exposed Person (PEP)?: Yes or No

Is the client, a member of their family, or a close associate of theirs the Head of an International Organization (HIO)?: Yes or No
Adding Attachments for POA
Please attach one file for each Executed Power of Attorney designated for this account
- Add File: Upload the file per POA.
Saving the Panel
After completing all required fields:
- Click Save to lock in the provided details.
- The panel status will update from Incomplete (yellow) to Complete (green) upon successful saving.
