How to Submit a Client Consent Form

You can submit a Client Consent Form request from a client’s CRM Contact Record as part of the onboarding process to authorize the sharing of their personal information with Optimize, NBIN, and related service providers. Use the instructions below to submit the form or request a signature from the client, ensure the form is signed, and upload it properly for compliance tracking.

How to Submit a Client Consent Form (Manual Submission)

Manual Submission is used when the Client Consent Form has already been completed and signed by the client outside the system. In this case, you simply submit the request and manually upload the signed document directly into the CRM. This method is ideal when the form has been physically signed or signed outside of the DocuSign workflow.

  1. Search for the client’s contact in the HubSpot Search bar and open the client’s CRM Contact record page.

  2. Navigate to the Servicing (Service Request) tab in the middle panel of the contact record.

  3. Click on "Client Consent Form".
  4. Click "Submit Client Consent Form".
  5. Select "Manual Submission".
  6. Select "Add File" to attach a signed Client Consent Form.
  7. Select "Submit" to submit the signed Client Consent Form.

How to Submit a Client Consent Form Request (Straight-Through Processing)

Straight-Through Processing (STP) is used when the form has not yet been signed by the client. This option automatically generates the Client Consent Form, then sends it to the client for electronic signature via DocuSign. Once signed, the form is automatically attached to the ticket and submitted for processing, reducing manual steps and ensuring a smooth onboarding flow.

  1. Search for the client’s contact in the HubSpot Search bar and open the client’s CRM Contact record page.

  2. Navigate to the Servicing (Service Request) tab in the middle panel of the contact record.

  3. Click on "Client Consent Form".
  4. Click "Submit Client Consent Form".
  5. Select "Straight-Through Processing".
  6. Enter the client's first and last name, as well as their e-mail.
  7. Click "Submit".

After Submitting the Request

A new service request ticket will be created. You can locate it:

  • In the Right-Side Panel of the client's CRM Contact Record under Tickets.
  • In the "Client Consent Forms" pipeline, where you can track the progress and confirm when the request is completed.

Processing Times

  • Typically 1 business day after the ticket has been submitted and completed review.
