Purpose of EMP5612
This form is used to transfer an RDSP from one financial institution (issuer) to another. The Registered Disability Savings Plan (RDSP) is a long-term savings plan designed to help Canadians with disabilities and their families save for the future.
When to use the RDSP Transfer Form
- When opening an new account the form is pre-populated through STP.
- When you submit an initiate a transfer manual.
What two forms needs to be completed
- Holder Consent to a Registered Disability Saving Plan (RDSP) Transfer
- Registered Disability Saving Plan (RDSP) Transfer
Where to find the forms
- Navigate to the Optimize Portal
- Select Documents
- Select Service Documents
- Type in RDSP. You will find both documents that are available for download.
The forms Include:

Information about the beneficiary (the person with the disability).

Details of the current RDSP (to be closed and transferred).

Details of the new RDSP (to be opened).

Authorization from the holder(s) of the plan.
